Please note that our office must be able to authenticate the origin of your immunization documentation in order for your records to be approved. There is a documents tab that you can use to upload your document as only one file. After you have your immunization records verified, please upload them to the Student Health Form for approval. Please review all of the instructions for the Student Health Form before filling it out. To find the Student Health Form, log into your ASIST account, click “Student”, select “Student Health Form”, complete the Immunizations, Medical History, & TB Questions sections. Immunizations and medical history must be documented through your Student Health Form. To maintain the health of our community, Liberty requires students to be immunized according to the Centers for Disease Control and Prevention (CDC) guidelines.
